Academic Plan Form (APF)

Students who plan to use any Chapter benefits need to submit an APF for each term. The OUR cannot certify a student without an approved Academic Plan Form for the enrolled term. 

Students utilizing the Cal Vet Fee Waiver ONLY do not need to submit an APF.

Two Ways to Submit Your APF

Complete & submit the APF through OASIS

  • Select the "Forms & Petitions" tab
  • Select "Submit a New Form"
  • Select "Academic Plan"
  • Instructions for how to complete and submit the APF are available on OASIS
  • The APF will need to be approved by your advisor*
    • *For students in the College of Biological Sciences: email cbsundergrads@ucdavis.edu to notify them of your APF submission for Veteran Certification
  • Print the Approved APF to PDF and upload through the OUR Veterans Portal

Complete the PDF Form

  • Annotate the correct courses (abbreviations) on the Academic Plan Form (e.g. PSY 100, STA 013, etc.). Please include the details of the course (virtual, hybrid, etc.) if applicable
  • Annotate the correct number of units for the course 
  • Ensure your Academic Advisor has reviewed the Academic Plan Form and has signed off on the designated signature line (digital signatures are accepted)
  • Ensure you have read and understood the form and sign it on the designated student signature line
  • Upload and submit the form through the OUR Veterans Portal
